Anna Q. Nilsson naît le 30 mars 1888 à Ystad, petite ville au bord de la Mer Baltique au sud de la Suède. En 1896, la famille s’installe à Hasslarp dans le sud-ouest suédois où son père travaille dans l’usine de sucre locale. Après sa scolarité, elle est engagée comme vendeuse dans une boutique de Halmstadt. Au milieu des années 1900, avec ses économies, elle embarque pour les Etats-Unis. Arrivée à New-York, elle est employée comme bonne d’enfants et apprend rapidement l’anglais. Quelques mois après son arrivée en Amérique, elle est remarquée sur une avenue new-yorkaise par le grand photographe Carroll Beckwith. En 1907, elle est déjà un modèle réputé posant pour de nombreux photographes de mode, des illustrateurs et des peintres. Trois ans plus tard, ses illustrations pour Penrhyn Stanlaws en «Stanlaws Girl», personnification de l’idéal féminin, la classe en tête des plus belles femmes des USA.
En 1911, Anna Q. Nilsson fait ses débuts au cinéma en vedette de «Molly Pitcher» une production de deux bobines tournée pour les studios «Kalem Company». Jusqu’en 1916, elle est l’interprète de quelques quatre-vingts court-métrages pour la «Kalem», principalement dirigés par Kenean Buel, où elle à pour partenaires: Miriam Cooper, Henry Hallam, Hal Clements et surtout Guy Coombs avec qui elle sera brièvement mariée en 1916. Pendant cette période, elle est surnommée «La folle d’Hollywood» car elle refuse d’être doublée dans ses films pour toutes les scènes périlleuses.
À partir de 1915, Anna Q. Nilsson est la principale protagoniste de productions plus importantes qui vont faire d’elle une star. Elle est alors l’interprète, entre autres, de: «Regeneration» (1915) l’une des toutes premières réalisations de Raoul Walsh, «The silent master» (1917) du français Léonce Perret, «The love burglar» (1919) avec Wallace Reid, «Le dictateur» (1919) avec Wallace Beery, «Son dernier exploit» (1920) avec William S. Hart , «L’île des navires perdus» (1923) de Maurice Tourneur, «La rançon d’un trône» (1923) de Cecil B. DeMille et «Terre maudite» (1925) de Frank Lloyd. En 1926, elle gagne vingt milles dollars par semaine, reçoit des milliers de lettres par mois et est élue la femme la plus populaire d’Hollywood. À ce titre, elle est la première actrice d’origine suédoise à recevoir son étoile sur le célèbre «Walk of Fame» de Hollywood Boulevard. Anna Q. Nilsson est aussi une femme d’affaire avisée, à son arrivée en Californie, elle achète un terrain et fait construire une maison au bord de la mer dans une endroit qui deviendra «Malibu» et investie dans des morceaux de désert dans la banlieue de Los Angeles qui seront le cœur de l’industrie cinématographique avec la construction des grands studios.
À la fin des années vingt, Anna Q. Nilsson est grièvement blessée à la suite d’une chute de cheval. Partiellement paralysée, elle part en Europe en convalescence et se fait soigner pendant plusieurs mois chez elle en Suède et en Autriche. Comme un malheur ne vient jamais seul, à son retour en Amérique, le cinéma parle. Elle s’éloigne alors des plateaux et ne revient qu’en 1933 pour un petit rôle auprès de Paul Muni dans «Le monde change» de Mervyn LeRoy. Elle tourne alors dans une quarantaine de films jusqu’en 1954 mais reste cantonnée dans des interprétations mineures parfois même des silhouettes. Seule est sans enfant, elle se retire dans sa propriété de Sun City. Anna Q. Nilsson meurt des suites d’une crise cardiaque, le 11 février 1974, dans une maison de convalescence d’Hemet, en Californie.
